As a new patient to our office, we will need to obtain demographic and insurance information from you when you arrive at your first appointment. Be sure to bring your current insurance card and photo ID with you to every appointment.

We file with the following insurance plans:

- Coventry
- Medcost
- Primary Physician Care
- Tricare Standard
- WellPath
You may contact our billing office at (336) 835-7700 during regular business hours to discuss any billing questions you may have.
Plans other than those listed above will be filed as a courtesy however payment in full is expected from the patient at the time of service.
Patients without insurance coverage are also expected to pay in full at the time of service.
If you are enrolled in a health plan with a deductible, we will ask that you make a deposit at the time of service.

Elkin Pediatric and Adult Medicine is fully compliant with the Health Insurance Portability and Accountability Act of 1996, also known as HIPAA. We will provide this notice to you when you check-in for your appointment.
